This one-page survey has pertinent questions and a rating scale that helps the company track customer service information. The judges said it addresses specific questions, such as how long did it take to respond to the client's initial call, did the estimator explain the contract, and was the jobsite left clean and safe each day. Most people don't take time to fill in the comments section, so the scale is the best way to collect pertinent information. The company logo and brand is used boldly at the top of the page. The judges also appreciated the inclusion of a self-addressed, stamped envelope and remind remodelers to always include one with any customer survey.
